Liberty Health Care Center nursing home provides long-term care services for those who are in need of 24-hour clinical/nursing care and assistance with personal care tasks that are often referred to as Activities of Daily Living (ADL). In addition to basic long-term care services, health education, social services, and medication management are also provided.
Specialized Skilled Nursing
Liberty Health Care Center specializes in therapeutic services such as Physical, Occupational, and Speech. We have an on-site Medical Director who rounds once a week; Nurse Practitioner who rounds 3 times a week; Certified Wound Care Physician who rounds once a week, a treatment nurse, and 24-hour nursing care (RN, LVN & CNA).
Quality of Life Managers are on site to provide life enrichment experiences. They always encourage our residents to stay active and involved socially, which keeps their moral high and speeds recovery.
Our facility offers similar benefits of a hospital, in a home like setting, to ensure a more comfortable recovery. We are designed for those in need of extra assistance in their recovery, with the goal in mind to become healthier and stronger before going back to their day-to-day lives.
Respite Care
Here at Liberty Health Care Center, we understand that caring for a loved one can become extremely overwhelming when you have to do it alone.
Our Respite care services are designed to give primary care givers a break by providing planned or customized care for individuals in need. This service is only for temporary situations; such as, going on vacation, holidays, moving or just in need of a few days to take a breather.
Respite care services are provided in our facility offering your loved one 24-hour nursing care.
Hospice Care
Here at Liberty Health Care Center, we know how difficult and emotional it can be for families to make the decision to place their loved one on Hospice Services. We also understand that, in some cases, not all our loved ones who are on hospice have a caregiver who can be with them 24-hours a day at home. We offer short and long-term placement for Hospice services and also contract with most hospice companies to ensure that our families and patients are well taken care of during such a sensitive time while in our facility.
