Sugar Land Health Care Center provides both short-term and long-term skilled nursing services for patients suffering from heart disease, COPD, diabetes, and various other diagnoses. Our experienced nursing staff monitor our residents’ health status, administer medications, and ensure that physicians’ orders are followed for other treatments. Furthermore, they coordinate the services and care of other health care providers, including specialists. In addition, a wound care nurse and a wound care physician are always on site. Our respite care services are designed to give primary care-givers a break by providing planned or emergency care for individuals in need. Our services are used for temporary situations and our duties include helping individuals with day-to-day activities and needs.
Our respite care services can be provided in-home, at a facility, or at a day center. We provide many options to allow flexibility with you and your schedule.
We offer short- and long-term care for those who have selected Hospice. Our nurses will coordinate with hospice care providers to ensure that the resident is well taken care of throughout the stay in our facility. We strive to provide compassionate quality care to support individuals and caregivers who are living with terminal illnesses no matter where they reside. We contract with Hospice providers that offer their services including on-call 24-hour support, clinical and skilled care, spiritual and emotional counseling. Our nurses will coordinate with hospice care providers to ensure that your loved one is well taken care of throughout the stay in our facility. This experience can be deeply meaningful and we strive to take the stress out of the process so you can enjoy all the little moments of life with your loved one.
